The Care Coordinator will provide care coordination services for identified high utilizers of Gateway services and will work closely with the detox, residential, outpatient, medical services, and other Gateway staff. The candidate will work to ensure that patients are provided with appropriate services to assist with meeting recovery and wellness goals. The candidate coordinates care across all of a patient’s areas of concern, including substance abuse, mental health, medical needs, housing, transportation, legal involvement, etc. The candidate will ensure services are provided appropriately and consistently to the patients being served, including providing outreach to identified high utilizers, and maintaining daily contact with patients enrolled in care coordination services. The candidate will coordinate admission to services and ensure smooth transitions and communication among providers. The candidate will provide assessment of patient needs across all life domains and will take a collaborative and patient-centered approach to care planning to help patients meet their individual goals. The candidate will provide transportation to patients when appropriate, and may provide outreach in the community, including meeting with patients in their homes when needed. The candidate needs to have a good understanding of the various services and programs offered at Gateway, as well as those offered in the surrounding community. The candidate will provide regular updates to the managing entity on patient engagement and services being provided and will participate in meetings with the managing entity and other community providers.
Must possess at minimum a bachelor’s degree in human services or a related field with 2 years’ experience in the field of substance abuse and mental health treatment. The candidate must have the ability to engage critical thinking and have a solution-focused mindset to assisting patients with complex needs. The candidate must have excellent oral and written communication skills, as well as the ability to provide high levels of customer service to patients, coworkers, community providers, and stakeholders. The candidate needs strong time management skills. This position will require the candidate to drive to various locations so they must have a valid state of Florida driver’s license with a safe driving record according to the standards of the organization’s insurance company. The candidate must be able to pass the organization’s driving test to drive a company vehicle to provide patient services in the community.
- Must be able to pass a level II DCF background screening
- Must be able to relate to adults, adolescents, families, referral sources, Gateway Team Members, community providers, and stakeholders and must possess requisite competencies to effectively understand and provide appropriate services to the patient population served.
