This is a professional position in which the incumbent will provide case management services working with a multidisciplinary team to address the patients’ needs. The incumbent must demonstrate competencies in population of substance abuse and mental health patients. The incumbent provides case management services that help patients gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. The case manager (1) provides an assessment of the eligible clients to determine service needs; (2) participates in development of an individualized integrated care plan; (3) provides referrals and related activities to help the patient obtain needed services; and (4) monitors progress and follows up on referrals. The incumbent provides feedback and documentation to the multidisciplinary team and referrals sources as well as documentation to the clinical record. The incumbent transports patients to appointments that are identified in the assessment and/or care plan. Incumbent will attend, provide and/or coordinate various education, training, and consultation for the program and the agency, as requested. Incumbent will be scheduled to work in the adult outpatient and residential substance abuse treatment programs that provide services to adults with behavioral health needs. Incumbent at times may provide evidenced based psycho-educational sessions in residential services. Incumbent will attend sessions and meetings as requested and assists the adult outpatient and residential teams as needed.
QUALIFICATIONS: Incumbent must possess a Bachelor’s degree in related field and 1 year experience in the case management of behavioral health and substance abuse disorders. The incumbent must have a valid state of Florida driver’s license with a safe driving record. Must be able to pass an agency driving test and be approved be approved by our agency insurance company for driving company vehicles. • Must be able to pass a level II DCF background screening • Must be able to perform assigned work independently with minimal supervision • Ability to relate to adolescents, adults, families, referral sources and internal Gateway Team Members and must possess requisite competencies to effectively understand and provide appropriate services to the client population served • Ability to maintain positive team approach • Ability to work in fast-paced environment under stress situations • Must have knowledge of the 12 step recovery process • Must have own reliable transportation • Must be able to complete required documentation of services within established timeframes and completeness
